Consumer feedback to the MoH

Published 15 September 2008, Posted in Northland Consumer feedback to the MoH

Disability Services of the Ministry of Health are requesting feedback from families and disabled people on the services they provide.

Most of these are the services that are received as a result of a Needs Assessment through your local NASC agency.

They have sent through the following questions for you to comment on.  If you have anything which you would like passed on to the Ministry please send this to me either by email or post by 24th September.

Thank you for taking to time to feedback your ideas.  This is our opportunity to influence change which will better support our needs as families.

Q1) What is working well?

Q2) What suggestions do you have, so that Disability Services might improve, for you and your family member?

Q3) Please describe any service gaps you have experienced.  Which of these do you think should be addressed as a priority?

Q4) Are there any other matters you wish to bring to the attention of Disability Services?

Please pass on to as many families as possible - Have Your Say!

Many thanks
